    Safety practices are not upheld.
    The presence of a toxic ‘lad’ culture amongst some staff and management.
    A failure to practice what they preach, senior management repeat a mantra of health and safety, then quality first, but in reality, production takes priority in the day to day running of things, until an operative makes a mistake due to time pressures or pressure from management, then they are told by management that they should be health and safety and quality focused!
